The primary responsibility of the The CMM is to administer medications in accordance with federal and state regulations and according to the Iowa State Department of Health under the direction of the delegating Registered Nurse.

 

Specific Tasks/Duties:

  •  Administers Medication According to Policy and Procedure
  • Washes hands prior to preparation and administration of medications
  • Starts medication pass at proper time.
  • Keeps cart in sight or locked at all times.
  • Keeps narcotic drawer locked at all times.
  • Knocks prior to entering room.
  • Identifies resident before administering medications.
  • Triple checks information on medication label with MAR for accuracy.
  • Obtains vital signs (if applicable) prior to administration of medications (pulse/BP).
  • Elevates head of bed before medication administration.
  • Gives medication by correct route.
  • Observes/ensures resident takes medication
  • Washes hands after each resident or uses hand sanitizer with hands washed after resident contact.
  • Does not borrow medications.
  • Completes medication pass in appropriate time frame.
  • Crushes medication according to policy and procedure
  • Assist residents with activities of daily living (bathing, dressing, grooming, toileting) and medication reminders, including identification of physical and cognitive changes, documentation, and communication to appropriate staff.
  • Assist residents with leisure time programming, including escorting them to and from activities.
  • Assist dining services with mealtime serving, clearing, and setting of tables.
  • Assist with resident laundry, including personal washing and bedding change.
  • Perform general housekeeping duties.
  • Perform other duties as assigned.

Position Requirements:

  • High school diploma or equivalent
  • Certified Medication Manger in the state of Iowa
  • Minimum of one year health care experience or two tears experience as a certified medication manager.
  • Proficiency in electronic health records.

Essential Functions:

  • Documents Medications and Vital Signs According to Policy and Procedure.
  • Medications are initialed after administration.
  • Controlled meds are signed out as administered.
  • Omitted and PRN doses are noted/charted/reported to the licensed nurse.
  • Nurse is notified prior to administration of PRN’s.
  • Vital signs are documented prior to administration of meds. ( B/P, pulse).
  • Controlled substance count is done at beginning and end of shift.
  • Discrepancies in controlled substances count are handled according to policy and procedure.
  • Assist with Resident Care and Unit Maintenance.
  • Assist with resident care as assigned.
  • Assist with obtainment of resident weights.
  • Ambulate residents as assigned.
  • Transports resident to activities.
  • Answers all resident call lights promptly.
  • Communicates – in clear, open, positive manner with peers, residents, families, physicians, and support staff.
